It also changes what gets documented. An adjuster valuing a file from records alone sees a diagnosis code and a bill total. They do not see the shift you could not cover, the season of coaching you had to hand off, or the stairs you now take one at a time. Those things belong in the claim, and they only get there if somebody asked about them early enough to gather the proof.
